Chief Warrant Officer Five Jamie D. Weeks died 14 May 2006 when his AH-6M Helicopter was shot down by enemy fire during combat operations in Iraq. He was born 13 March 1959 and graduated high school in Mons, Belgium. He earned his Bachelor of Science Degree (Cum Laude) in Professional Aeronautics from Embry Riddle Aeronautical University in 2005.

CW5 Weeks volunteered for service in the U.S. Army in 1980 and served 38 months as an Air Traffic Controller before attending Warrant Officer Candidate School in 1983. He completed flight training as an AH-1 Attack Helicopter Pilot in October 1983. His first assignment was with 9th Battalion, 4th Aviation Regiment at Fort Hood, Texas, where he served as an AH-1 Pilot. In 1986, he was assigned to Camp Stanley, Korea, where he served as an AH-1 Pilot. Upon completing his tour in Korea, CW5 Weeks was assigned to Fort Campbell, Kentucky, where he served as an AH-64 Attack Helicopter Pilot in 1st Battalion, 101st Aviation Regiment. While at Fort Campbell, he deployed to Operation DESERT SHIELD/STORM in 1990 as an AH-64 Instructor Pilot. In 1993, he assessed with the 160th Special Operations Aviation Regiment (Airborne). Upon completion of Green Platoon, he was assigned as an A/MH-6 Instructor Pilot. While assigned to the 160th SOAR(Abn), CW5 Weeks deployed to Haiti in 1994 in support of Operation RESTORE DEMOCRACY. His assignments in the 160th SOAR(Abn) include Instructor Pilot, Instrument Flight Examiner, Company Safety Officer, and Standardization Instructor Pilot. After 9/11, CW5 Weeks deployed on multiple rotations in support of Operations ENDURING FREEDOM and IRAQI FREEDOM.

CW5 Weeks was a graduate of the Survival, Evasion, Resistance, and Escape (SERE) (High Risk) Course, Air Assault School, Rotary Wing Instructor Flight Examiner Course, Warrant Officer Senior Staff Course, Joint Firepower Controller Course, Aviation Safety Officer Course, AH-64 Instructor Pilot Course, OH-6 Instructor Pilot Course, and Aviation Life Support Specialist Course.

CW5 Weeks' awards and decorations include the Meritorious Service Medal (w/1 OLC), Air Medal (w/"V" Device), Air Medal, Army Commendation Medal (w/2 OLC), Army Achievement Medal, Presidential Unit Citation, Joint Meritorious Unit Award (w/1 OLC), Army Valorous Unit Award, Army Good Conduct Medal, National Defense Service Medal (w/1 Bronze Service Star), Armed Forces Expeditionary Medal, Southwest

Asia Service Medal (w/1 Bronze Service Star), Iraq Campaign Medal, Global War on Terrorism Expeditionary Medal, Global War on Terrorism Service Medal, Korean Defense Service Medal, Army Service Ribbon, Overseas Service Ribbon, Kuwait Liberation Medal (Saudi Arabia), Kuwait Liberation Medal (Government of Kuwait), Combat Action Badge, Master Army Aviator Badge, and the Air Assault Badge. He was posthumously awarded the Silver Star, Legion of Merit, and Purple Heart.

CW5 Weeks is survived by his wife, Robin; four daughters, Lauren, Courtney, Lindsey and Emily; his mother, Glenda; and four grandchildren, Parker, Jackson, Jamie Rose, and Zachary. He is laid to rest in Clarksville, Tennessee, at Resthaven Memorial Gardens. The Regiment Headquarters and Headquarters Company office, building #7259b, is dedicated in his honor as Weeks Hall.
